ROW reserves the right to collect personal identifying information on individuals who are using our services. This information will not be disclosed to any third party with the following exceptions:
- Reason to believe that child abuse/neglect has occurred
- The intent to harm self or others
- For funding/auditing purposes
- Request via a Court Order
- We have a signed release by our client to share information
Our ability to assist you depends on having certain personal identifying information. If you choose not to share the information we request, we reserve the right to decline to provide you with services, as doing so could jeopardize our status as a service provider. We assume that, by requesting services from our agency, you agree to allow us to collect information and to use it or disclose it as described in this notice and otherwise as allowed or required by law.
Confidentiality Rights: Maintaining the privacy and safety of those using our services is very important to us. This agency follows all confidentiality regulations and also has its own confidentiality policy. As a individual receiving services through Redevelopment Opportunities for Women, Inc. (ROW), you have the following rights:
- To humane care and treatment
- To receive prompt evaluation and care
- To be involved in the development of your goal/action plan
- To be treated with dignity and respect
- To have your records kept confidential
- To have the same legal rights and responsibilities as any other citizen
- To be free from verbal and physical abuse
- To be free from sexual harassment
- To see and review your own records and to request a correction of your record
- To file a grievance if you feel that you have been unjustly served, put at personal risk, harmed, or your personal information was not handled correctly
